Before you go to campus, please document your health status.

IF YOU HAVE ANY SYMPTOMS OF COVID-19, PLEASE CONTACT A MEDICAL PROFESSIONAL.

PLEASE NOTIFY YOUR INSTRUCTOR IF YOU HAVE BEEN DIAGNOSED WITH COVID-19. IF YOU HAVE BEEN TOLD TO SELF-QUARANTINE BASED UPON YOUR EXPOSURE TO SOMEONE WITH COVID-19, PLEASE LET YOUR INSTRUCTORS KNOW AND FOR HOW LONG.

Before leaving your home to go to Community Colleges of Spokane (CCS), Spokane Community College (SCC), or Spokane Falls Community College (SFCC), review the symptom list below and answer the following questions.

If you plan on visiting somewhere besides your classroom(s) when you are on campus and will be in close contact with other people, which means closer than six feet away for more than 15 minutes, please enter those locations below.

- Please notify your instructor that you will not be attending class and why.
- You are encouraged to be tested for COVID-19.
- If you do not have a doctor or healthcare provider, free or low-cost testing is available at several locations in Spokane to anyone, regardless of immigration status. If you live in a different county please click here for testing locations.
- You may return to school once you have completed a quarantine or isolation period or if you provide a statement from a medical professional stating that you do not have a contagious illness and the symptoms you exhibited are not because of COVID-19.
If you answered “NO” to all questions then you may attend school as planned.
