Step 1

  • Log into the ctcLink Employee Homepage

  • Click the Immunization Attestation tile

  • Click "Immunization" dropdown menu 

  • Select the Immunization or Exemption Type from the drop-down list and input the date received

  • If applicable, to enter second dose information, click the [+] icon then select Immunization Type and Date Taken

  • Answer the Self Attestation section statement by clicking on the slider to display, "Yes"

  • Answer the Disciplinary Action section statement by clicking on the slider to display, "Yes, I agree"

  • Verify all information is accurate and then click Submit to save

  • Information will be saved, but you will stay on the page.  Click the ctcLink Employee Homepage link at the top of the page to return to the Employee Homepage

  • Updates can be made by returning to the Immunization Attestation tile in the ctcLink Employee Homepage as needed
